Other course forms:
- Extra Credit form - Students who take private music lessons or participate in a regularly scheduled, extra-curricular performing ensemble, such as YPSO, are eligible for extra credit. Fill out the form and have your teacher or ensemble director sign it, then turn it in to Mr. Benstein by the end of each semester.
- Performance Review form - Use this form to write a review of a performance you attend. Students are required to attend one performance per quarter.
